Intensive Home Based Therapy (IHBT) Counselor

At Appleseed Community Mental Health Center, Inc., we walk with families and individuals on their journey to live healthy, whole, and productive lives by offering comprehensive and integrated behavioral health, housing, outreach & prevention, substance use disorder, and victim services. To help accomplish this goal Appleseed is seeking a highly skilled Intensive home based therapist/social worker (IHBT) to join our team full time. Must have a car available during working hours, carry and maintain automobile insurance that includes a payable liability of $100,000/$300,000, as well as the ability to legally drive a car. Must pass background and driving record checks and a drug test.

Requirements

  • Experience working with youth, families, and/or groups around behavioral, emotional, and cognitive issues required.
  • Applicant must demonstrated knowledge of mental illness, motivational interviewing, family systems treatment, trauma informed care, and crisis interventions, and knowledge and skill in the use of the Diagnostic and Statistical Manual of Mental Disorders V/ICD-10.
  • Applicant must also have strong commitment to client rights.
  • Must be licensed, minimum of Bachelors level

Responsibilities

  • Participate in individual and group supervision
  • Provide IHBT services in the client’s home and in community
  • Average a caseload of 4-6 (or less) youth/families at a time, averaging 3 service hours per with, with each case averaging 3-6 months
  • 24/7 on-call for immediate response to crisis calls, with face to face response available as needed by IHBT staff assigned to case, with IHBT program staff covering on-call on weekends and holidays
  • Assess, monitor, and review safety concerns, risk behaviors, escalation patterns, crisis triggers, develop individualized safety plans with actionable crisis stabilization steps, incorporating natural supports of each IHBT youth/family
  • Use comprehensive and integrated trauma informed mental health treatments (crisis response, stabilization, safety planning, cross-system collaboration, advocacy, family counseling, individual counseling, skill building, resilience promotion, support enhancement)
  • Use system of care best practices including culturally, developmentally and linguistically appropriate, trauma informed, resiliency-focused, strength-based, youth guided, family-driven, individualized to the unique needs and strengths of the youth and family
  • Utilize a structured format for case review along with input from Supervisor
  • Complete IHBT Core Training within 6 months of employment start date
  • Participate in individualized training plan based on an assessment of specific training needs, including quarterly trainings specific to any identified training needs
  • Collect consumer satisfaction feedback utilizing IHBT surveys
  • Participate in a “ramp up” period of up to four months prior to seeing clients (promote program, attend trainings and supervision, build relationships with partners, etc)
